Privacy policy

§ 1 General principles

  1. This Privacy Policy sets out the rules for the collection, processing and use of personal data obtained through the website psychologkostusiak.pl (hereinafter: the “Website”).
  2. Using the Website does not require the provision of personal data, unless the User intends to purchase a product or send an enquiry.
  3. The provision of personal data by the User is always their voluntary decision. However, a lack of consent to the processing of personal data may make it impossible to provide services by electronic means.
  4. The controller of the personal data provided by Users is NDI GROUP NATALIA KOSTUSIAK, with its registered office in Komorniki, ul. Stawna 29F, 62-052, tax identification number (NIP) 9720982409.
  5. The Controller protects personal data against disclosure to unauthorised persons, loss, alteration, damage or destruction, and processes the data entrusted to it in accordance with the provisions of law. The legal basis for the processing of personal data is Article 6(1)(b) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data (GDPR).
  6. While using the Website, additional information may be collected, in particular: the IP address assigned to your computer or the external IP address of your Internet provider, the domain name, the type of browser, the time of access and the type of operating system.
  7. Navigation data may also be collected from Users, including information about the links and references they choose to click or other actions taken on the Website.

§ 2 Rules for processing personal data

  1. Personal data is processed on the basis of the consent given by the User of the website when using the contact form on the Website.
  2. The personal data referred to in § 2 paragraph 1 includes, above all:
    • first name and surname,
    • telephone number,
    • email address.
  3. In the case of a User running a business, the User, apart from the data referred to in paragraph 2 above, also provides the following personal data:
    • the business name under which the enterprise operates, together with an indication of its legal form,
    • the entry number in the Central Register and Information on Economic Activity or the entry number in the National Court Register,
    • the address of the registered office,
    • the correspondence address,
    • the tax identification number (NIP),
  4. The personal data provided by the User will be processed for the purpose of providing services by electronic means via the Website and responding to an enquiry made in the contact form.
  5. The User has the right to access, control and verify the data concerning them that is being processed, and to obtain information about the purpose, scope and manner of processing of the data contained in the data set, and to demand that personal data be supplemented, updated or rectified, that its processing be temporarily or permanently suspended, that data be erased, that the processing of personal data cease on the basis of Article 32(1)(7) of the Personal Data Protection Act, or to lodge an objection in accordance with Article 32(1)(8) of the Personal Data Protection Act.

§ 3 Basic rights of the User

  1. The User may at any time request the erasure of their data from the database of NDI GROUP NATALIA KOSTUSIAK by sending such a request by email to kontakt@psychologkostusiak.pl, preferably from the email address provided during registration.
  2. The erasure of the User's data results in the deletion of all information and data about the User. However, the administrator of the website www.psychologkostusiak.pl reserves the right to retain the User's data and to refuse to erase it to the extent and for the period permitted by law, in particular in order to settle obligations arising between the User and the Company.

§ 4 Basic obligations of the data Controller

  1. NDI GROUP NATALIA KOSTUSIAK, as the controller of personal data, takes particular care in the process of data processing in order to protect the interests of Users.
  2. NDI GROUP NATALIA KOSTUSIAK ensures that personal data is:
    • processed lawfully,
    • collected only for specified, lawful purposes,
    • not subjected to further processing that would be unlawful,
    • adequate and sufficient for the purposes of processing,
    • stored only for the necessary period, i.e. until the purpose of processing has been achieved,
    • stored in accordance with the highest security standards,

  3. Personal data is stored in a data set that is protected by technical and organisational measures ensuring the protection of data processing. Access to the personal data set is granted only to persons authorised by the personal data controller.

§ 5 Cookies mechanism, IP address

  1. The Website uses small files called “cookies”. They are saved by NDI GROUP NATALIA KOSTUSIAK via the Website on the computer of a person visiting the Website, if the web browser allows it. A cookie usually contains the name of the domain it comes from, its “expiry time” and an individual, randomly selected number identifying the file.   5. The cookie mechanism is safe for the computers of Users of the Website. In particular, it is not possible for viruses or other unwanted or malicious software to get onto Users' computers by this route. Nevertheless, Users can limit or disable the access of cookies to their computers in their browsers. If this option is used, it will be possible to use the Website except for the functions which by their nature require cookies.
